Charlotte Hungerford Hospital is a 122-bed, general acute care community hospital located in Torrington, Connecticut, that serves as a regional health care resource for the 100,000 residents of Litchfield County and Northwest Connecticut. Job Summary: Responsible for providing patients referred to cardiac rehabilitation services in outpatient settings with supervised exercise and progressive mobilization, education regarding disease and disease management (including an understanding of community resources upon discharge), lifestyle modification, and psycho-social support. The Exercise Physiologist uses an understanding of the principles of growth and development to assess the patient’s age specific needs and age specific treatments and care.
Develops appropriate and comprehensive treatment plans specific to patient needs, including education, exercise, behavior change, and counseling
Provides patient testing and assessment before and after rehabilitation sessions.
Monitors patient/participant progress and communicates and coordinates care Supervises exercise, monitors responses, and directs care accordingly among health care disciplines/providers.
Documents treatments completely and succinctly in patient charts, reports noteworthy findings to appropriate health care providers.
Maintains accurate patient program data and assists in outcomes measurement/management
Provides patient education to individuals/groups in a clear, well organized fashion.
Qualifications
Qualifications:
B.S. in Exercise Physiology/Science - Master's Degree preferred
Basic monitor interpretation skills. Must successfully pass dysrhythymia class within 3 months of hire
Certified BLS provider, credentialed from the American Heart Association or American Red Cross
ACSM Clinical Certification preferred
Cardiac Rehab experience preferred
