Job Summary:
Charter Oak Health Center is seeking a Triage Nurse (RN) to join our team. In this role you would be responsible for assessing patients' conditions, determining the urgency of their needs, and ensuring they receive appropriate care promptly. This role involves conducting initial evaluations, providing medical advice, and coordinating with healthcare providers to prioritize patient care.
Key Responsibilities:
- Conduct initial assessments via phone or in-person to evaluate the urgency of patients' conditions.
- Determine the level of care required and coordinate with healthcare providers to prioritize patient treatment.
- Provide medical guidance and advice based on assessments.
- Record and maintain accurate patient information and assessment findings.
- Identify and address potential emergency situations promptly.
- Educate patients on health management and follow-up care.
- Work with healthcare team members to deliver optimal patient care.
- Assist in developing and updating triage protocols and procedures.
Qualifications:
- Bachelor's degree in nursing (BSN).
- Active Registered Nurse (RN) license in the State of CT.
- Minimum of three years' experience preferred ER/ICU/Med/Surg.
- Strong clinical and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment.
- Proficiency in using electronic medical records (EMR) systems.
- Certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
