Job Summary:
Charter Oak Health Center is seeking An Urgent Care Nurse Practitioner (APRN) to join our team. In this role, you will be responsible for providing high-quality, immediate care to patients with non-life-threatening conditions. This role involves diagnosing and treating a variety of illnesses and injuries, performing minor procedures, and collaborating with other healthcare professionals to ensure comprehensive patient care.
Key Responsibilities:
- Conduct thorough assessments of patients' health conditions, including medical history and physical examinations.
- Diagnose acute illnesses and injuries, develop treatment plans, and prescribe medications as needed.
- Perform minor medical procedures such as suturing, wound care, and splinting.
- Order, interpret, and follow up on diagnostic tests such as X-rays, EKGs, and lab work.
- Educate patients and their families on treatment plans, preventive care, and healthy lifestyle choices.
- Work closely with physicians, nurses, and other healthcare professionals to provide coordinated care.
- Maintain accurate and detailed patient records, including treatment plans and progress notes.
- Provide immediate care in emergency situations and stabilize patients for transfer if necessary.
Qualifications:
- Master's degree in nursing (MSN)
- 1 to 2 years' experience in ambulatory care setting preferred, however new graduates are welcome to apply.
- Active Nurse Practitioner (NP) license and certification as an Advanced Practice Registered Nurse (APRN) in the State of CT.
- Strong clinical and diagnostic skills.
- Excellent communication and interpersonal skills.
- Proficiency in using electronic health records (EHR) systems.
- Certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
