Charter Oak Health Center is seeking a Bilingual Therapist (Fluent Spanish) to join our team. In this role, you will be responsible for conducting assessments, developing treatment plans, and providing therapy in both Spanish and English. The ideal candidate will have a strong background in providing culturally competent mental health and behavioral health services to a diverse patient population, including underserved and vulnerable communities. Key Responsibilities :
Clinical Services
- Conduct comprehensive mental health assessments to determine patient needs and appropriate interventions..
- Provide evidence-based therapy (individual, group, and / or family) in Spanish and English.
- Monitor patient progress and adjust treatment plans as needed.
Collaboration and Integration
Work closely with medical providers, social workers, and other staff to ensure integrated care for patients.Participate in team meetings and case consultations to develop comprehensive care plans.Refer patients to internal and external resources, including psychiatric services and community-based programs.Documentation and Compliance
Maintain accurate and timely documentation of all patient interactions in the electronic health record (EHR) system.Ensure compliance with FQHC policies, HIPAA regulations, and other applicable standards.Participate in quality improvement initiatives and data collection to support program evaluation.Professional Development
Attend training and professional development opportunities to maintain licensure and enhance clinical skills.Stay informed about best practices in mental health treatment and integrated care models.Compliance Responsibilities
Complies with applicable legal requirements, standards, policies and procedures including but not limited those within the Compliance Process, Code of Conduct, HIPAA and Corporate Integrity Agreement (CIA).Participates in required orientation and training programs, as required.Cooperates with monitoring and audit functions and investigations.Participates, as requested, in process improvement responsibilities.Requirements : Qualifications :
Master’s Degree in Social Work (LMSW, LPC-A, LCSW, or LPC) requiredMinimum 3 years’ experience as a Licensed Clinical Social Worker in outpatient setting; experience running therapy groupsFluent Spanish is requiredExperience working with underserved populations or in community health settings is highly desirable.Familiarity with integrated care and FQHC models of care.Proficiency in electronic health record (EHR) systems.Charter Oak Health Center Offers Outstanding Benefits That Include :
