Field Coordinator

Full Time: 40 hours a week (Monday to Friday 8am-5pm)

We are seeking a compassionate and empathetic candidate experienced working with the elderly in the home care field to join our fast-growing team.  The candidate will act as a liaison with the office team, clients, and client’s care team by going out on home visits to help ensure quality care services.  The Field Coordinator will provide personalized care to clients assisting in specialized activities and overall care management for clients in their homes.  The position will be a back-up care provider as needed during times when care-team members are not available.

The Field Coordinator will communicate with the office team as needed.  This position will provide training and demonstration of skills to other caregivers as assigned.  This position will report directly to the Office Manager to ensure that all clients are being taken care of to the standards of Charter Oak Home Care and follow through with any other duties required. This position may also include conducting initial care consultations or follow up assessments of clients as needed.  From time to time, this position may also assist with distributing marketing materials and providing administrative support to the office team.  This position is full-time during the week and the candidate must be flexible to work with clients during the allotted work hours.

General Qualifications:

• Experience assisting clients with personal care activities to include personal hygiene, mobility assistance, light housekeeping, feeding and meal preparation
• Experience conducting/creating training programs
• 2 years of experience in home health/home care/ healthcare field
• Demonstrable ability to exude compassion and empathy is required
• Possess reliable transportation and a valid driver’s license
• Ability to meet the job requirements for reliability during inclement weather
• Alzheimer's or Dementia care  experience is preferred 
• CNA/HHA/PCA certifications are preferred.

Responsibilities

• Deliver exceptional care to dedicated client in their home
• Help execute comprehensive personalized weekly schedules and care plans
• Supply personal companionship, remain actively engaged with each client by providing ongoing social & emotional support
• Assist with personal care activities to include personal hygiene, mobility assistance, light housekeeping, feeding and meal preparation
• Keep order, safety and client enjoyment top of mind
• Provide transportation to and from appointments as needed

• Administrative functions such as documentation of completion of tasks and report changes in the client’s condition

 Requirements

• Compassion and empathy is a must
• Alzheimer's or Dementia experience is a plus
• Ability to use good judgement and remain calm
• Reliable and on time
• Proof of eligibility to work in U.S.
• Can pass a background check and provide references

Salary : $20 - $23
