Resident Care Coordinator (RCC) Position Summary :

The Resident Care Coordinator (RCC) ensures the quality care of all residents. This position is responsible for promoting and  integrating the mission and core values of Charter Senior Living. The RCC manages the care staff in AL by scheduling, evaluating, and supporting the frontline care  staff while reporting to the HWD. The Resident Care Coordinator is responsible for overseeing the care and wellbeing of residents in a senior living community. This role involves assessing, planning, coordinating, and monitoring individualized care plans for each resident while ensuring that the community maintains a high standard of care, service, and safety. The coordinator serves as a liaison between residents, their families, healthcare providers, and the staff to deliver personalized, quality care in a supportive, compassionate, and resident-centered environment.

Key Responsibilities :

Care Plan Development & Implementation :

Conduct thorough assessments of new and existing residents to develop individualized care plans based on their medical, physical, and emotional needs.

  • Regularly review and update care plans to ensure they meet the evolving needs of residents, working with interdisciplinary teams as needed.
  • Ensure that care plans are communicated effectively to all relevant staff members, including nursing, activities, and dietary teams.

Coordination & Supervision :

Oversee the day-to-day delivery of care services to ensure quality, consistency, and compliance with company policies and healthcare regulations.

  • Coordinate with medical professionals, caregivers, and other healthcare providers to ensure timely and appropriate interventions.
  • Assist with the scheduling of in-house medical appointments and services (e.g., physical therapy, home health aides, and doctor visits).
  • Monitor the physical and emotional health of residents, and take appropriate action in case of changes in condition or emergencies.
  • Resident & Family Engagement :

    Act as a point of contact for families and residents, providing updates and addressing concerns about care.

  • Develop strong relationships with residents and their families to ensure satisfaction and promote a positive living environment.
  • Facilitate regular family meetings to discuss care plans, health status, and any needed adjustments to care.
  • Offer emotional support and guidance to families regarding aging and care-related matters.
  • Compliance & Quality Assurance :

    Ensure adherence to federal, state, and local regulations regarding senior care and senior living standards.

  • Conduct regular audits of care plans and care services to maintain compliance and improve quality of care.
  • Participate in staff training programs to maintain high standards of care and provide ongoing education about resident care.
  • Maintain accurate and timely documentation of resident assessments, care plans, and updates as required by the facility and regulations.
  • Staff Support & Leadership :

    Provide guidance and mentorship to care staff, ensuring they have the resources and training necessary to meet the needs of residents.

  • Lead by example to promote a team-oriented and compassionate approach to resident care.
  • Assist with recruiting, hiring, and evaluating care team members.
  • Emergency Response :

    Act as a key member of the emergency response team, ensuring that residents receive appropriate emergency medical care when needed.

  • Provide crisis management and intervention as necessary in accordance with company policies and procedures.
  • Qualifications : Education :

    Bachelor’s degree in Nursing, Social Work, Gerontology, or a related field preferred.

  • Certification in Nursing (RN / LPN), CNA, or other relevant healthcare qualifications may be required based on state regulations.
  • Experience :

    Minimum of 3-5 years of experience in senior living or healthcare, with at least 2 years in a leadership or supervisory role.

  • Previous experience coordinating care plans and managing teams in a senior living environment preferred.
  • Skills & Abilities :

    Strong interpersonal and communication skills, with the ability to build relationships with residents, families, and staff.

  • Knowledge of healthcare regulations and best practices in senior living care.
  • Excellent problem-solving, organizational, and time-management skills.
  • Ability to work under pressure and handle emergencies with composure.
  • Empathetic, patient, and passionate about working with seniors.
  • Certifications :

    CPR and First Aid certification, or the ability to obtain certification.

  • State-specific LPN licensure requirements apply.
  • Working Conditions :

  • The position may require standing, walking, and lifting up to 25 pounds.
  • Must be available to work evenings, weekends, and holidays as needed.
  • Exposure to various environmental conditions, including but not limited to physical demands related to caregiving and healthcare duties.
