GENERAL SUMMARY
Responsible for participant (member) intake, assessment and care plans, caregiver communication and facilitating the support group. Facilitates program development, as it relates to maintaining therapeutic activities, proper staff to member ratio, and overall daily operation staffing and coordinates administrative tasks with the BTSS Director.
ESSENTIAL DUTIES & RESPONSIBILITIES

- Creates intake assessments including specific information about participant preferences and abilities; maintains ongoing assessment of needs.
- Organizes care-planning meetings to maximize client/family involvement.
- Ensures that activities programs are individualized for each participant.
- Oversees the development of a comprehensive activities program to meet the needs of participants with Alzheimer’s disease and other dementia disorders.
- Orients all staff to psychosocial needs of members. Communicates with other service providers as needed.
- Maintains strong family/caregiver communications including facilitating monthly forums.
- Recommends staff development and training opportunities to Director for all staff positions.
- Conducts program tours for new family members and other interested parties in coordination with staff. Maintains inquiry/interest list for follow up.
- Works collaboratively with staff, volunteers and invites guests to incorporate their specialized experience into activities programming.
- Maintains charting, correspondence, program statistics and required reports as needed.
- Elicits feedback from participants and family members to enhance activities programming.
- Assists program care assistants in conducting program activities and in providing personal care, as needed.
- Meets weekly with Office and Activity Assistant to confirm staff schedules and review upcoming activities.
- Facilitates program development and ensures proper staff to member ratio is maintained.
- Facilitates monthly support group for community members seeking support while caring for someone with Dementia or a related illness.
- Performs other duties as required.
MINIMUM ENTRANCE QUALIFICATIONS
Education and Experience
- Bachelor's Degree from an accredited college with major course work in Social Work or related field
- 3 to 5 years' dementia specific experience
- Any equivalent combination of training, education, and experience that provides the required skills, knowledge and abilities
