You may be just the right candidate for this or other roles.Purpose of Job : Improve the overall health of the communities we serve by supporting patients with complex needs to achieve their health goals and improve health outcomes as follows : Essential Duties and Responsibilities : Identifies and evaluates high-risk patients through available reporting.Works collaboratively with patients, within CHAS Health multidisciplinary teams, and outside entities, to facilitate effective transitions of care for hospitalized patients.Leads efforts to coordinate care and perform case management with patients to improve their health outcomes through creating plans to improve chronic conditions and achieve more effective personal, social, educational and vocational development.Performs and / or facilitates patient, family, and caregiver education and assists with patient-centered health action plans.Responsible for tracking and presenting outcomes for assigned caseload to leadership.Collaborates with other resources within organization to arrange housing, transportation and other social determinants of health for patients with such needsProvides or facilitates at-home visits for patients as needed.Performs other duties as assigned, including supporting the CHAS Health Mission and Core Values.Qualifications : Education / Experience : 3-5 years of experience within a community health setting preferred. Experience with case management is preferred. Experience with electronic health records preferred. Licensed Social Worker : Master's in social work from an accredited university required. Valid state licensure as an Independent Clinical Social Worker or Licensed Advanced Social Worker. LCSW-A or LASW-A with equivalent case management experience will be considered. Other Licensures- CDP, SUD-P or equivalent licensure with patient populations. Bachelor's degree in related field with two years experience. Certification in Case Management required at time of hire.Skills : Excellent customer service, oral and written communications skills required. Compassion for patients required. Must possess a valid driver's license and proof of insurance. Must be able to work well in a team environment. Ability to multi-task in fast-paced work environment, with ability to anticipate future needs required. Computer skills required. BLS (CRP / AED) required. Precepts students as needed. Valid drivers' license and insurance required. Commitment to supporting a safe, respectful, equitable, and inclusive environment required. Physical Demands : While performing the duties of this job, the employee is required to stand, sit, and be mobile one-third to two-thirds of the time. They are required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms. Climbing or balancing occurs less than one-third of the time; while stooping, kneeling or crouching occurs between one-third and two-thirds of the day. Communicating by talking / hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 pounds. Rarely is there a need to lift more than 26 pounds.
