Pay Range:
$79,998 - $110,722 USD Annually - Independently Licensed Clinical Social Workers; Mental Health Counselors; Marriage and Family Therapist
$32.50 - $47.73 USD Hourly - Licensed Social Worker Associate; Mental Health Counselor Associate, Marriage and Family Therapist Associate  
Salary is determined based on applicable experience and education. External offers typically do not exceed the mid-point of the pay range.

Purpose of Job:

Improve the overall health of the communities we serve by providing social or behavioral health services to patients and families by performing the following duties:

Essential Duties and Responsibilities:

•Provides individual and group counseling services within individual scope of practice to assist in clients in achieving more effective personal, social, educational and vocational development and adjustment. 

•Available to primary care providers for behavioral health staffing and consultation. 

•Collects psycho-social data about individuals and provide gathered information to providers to assist them in managing medical care. 

•Perform mental health evaluations utilizing approved evaluation tools assessing amenability to mental health treatment and level of care needed. 

•Develops detailed care plans for each client and document as part of the electronic health record.

•Links clients with various community resources. 

•Provides in-service training for CHAS staff as requested. 

•Participates in community outreach activities as directed. 

•For staff with chemical dependency expertise:

o Responsible for performing chemical dependency assessments on clients using approved testing instruments and evaluating tools, and determining their amenability for referral to other agencies and/or programs.

o Conducts individual and group counseling and education focusing on alcohol and drug treatment, relapse prevention, and cognitive behavioral change.

•For staff with clinical supervision training and duties:

o Supervises and coordinates volunteers and behavioral health / social work students as required.

o Facilitates peer review and chart audit process as assigned.

• Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.

Qualifications:

Education/Experience: Valid state licensure as a Licensed Advanced Social Worker (or associate), Licensed Independent Clinical Social Worker (or associate), Licensed Mental Health Counselor (or associate), Licensed Marriage and Family Therapist (or associate), Licensed Clinical Professional Counselor, and/or Licensed Professional Counselor required. Two years of clinical practice with underserved populations desired.

Skills: Knowledge of available community linkage resources. Excellent oral and written communications skills required. Computer skills required. Work requires consistent exercise of independent judgment and discretion. BLS (CPR/AED) required.  Valid drivers’ license and insurance required. Commitment to supporting a safe, respectful, equitable, and inclusive environment required.

 

Physical Demands:

Movement around the facility between 1/3 and 2/3rds of the day is required, as well as sitting at a workstation or patient rooms. Using hands occurs over 2/3rds of the day, while reaching with arms less than 1/3rd of the day. Communicating is required over 2/3rds of the day. Lifting up to 10 lbs occasionally is required.
