This position functions as a Social Worker II in the Adult Services Unit and Family Services Division of the Chatham County Department of Social Services. The Adult Services Unit is responsible for all adult services functions and programs including adult protective services, guardianship, special assistance in-home services, adult care facility monitoring, and various general case management services. Adult Services focuses on the restoration and maintenance of the client’s physical health, emotional well-being, safety, social and economic functioning. The Social Worker II provides indirect and direct service provision to Chatham County’s aging adults and adults with disabilities and completes various tasks to assist the Social Worker IIIs in the unit.
  • Evaluate clients, their support systems, and their ability to manage activities of daily living; develop person-centered service plans and provide follow-up as needed;
  • Receive APS reports, perform outreach visits, provide follow-up on information and referral calls; assist with locating relatives;
  • Make required monthly and collateral contacts, provide individual family adjustment services, assist with transportation needs, and complete various tasks associated with short-term case management;
  • Thorough and timely documentation of case planning and social work activities in the case record with adherence to local and state timeframes and guidelines;
  • Build and maintain partnerships with community agencies and represent the unit by participating in local and statewide initiatives and committees;
  • Assist with the development of and participate in all unit outreach events;
  • Serve as backup for intake services during office hours;
  • Participate in the rotation for APS on-call with the adult services team;
  • Participate in shelter operations or other weather-related incidents as needed;
  • Performs other duties as assigned.
Bachelor's degree in social work from an appropriately accredited institution; Bachelor's degree in a human services field from an appropriately accredited institution and one year of directly related experience; Bachelor's degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.
  • Thorough knowledge of social work principles, techniques, and practices, and their application to complex casework, group work, and community problems;
  • Considerable knowledge of a wide range of medical, behavioral, and/or psychosocial problems that individuals may be experiencing;
  • Considerable knowledge of family and group dynamics and a range of intervention techniques, governmental and private organizations, and resources in the community, laws, regulations, and policies that govern the program;
  • Considerable knowledge of medical terminology, disease processes, and their treatment as they relate to decisions regarding clinical interventions and appropriate therapies based on medical or psychological diagnosis;
  • Awareness of needs and challenges associated with the aging and disabled population and the ability to navigate community resources.
  • Ability to plan and coordinate;
  • Ability to deal tactfully with others and to exercise good judgment in appraising situations and making decisions;
  • Ability to be flexible and to work as a team player;
  • Ability to demonstrate good verbal and written communication skills;
  • Ability to demonstrate excellent listening skills;
  • Ability to demonstrate good public relations skills;
  • Skill in operating standard office equipment such as computers, phones, fax machines, and scanners; 
  • Ability to perform work using local and state technology systems including NCFAST, Northwoods, iiReception, Microsoft Office, internet browsers, virtual meeting platforms such as Microsoft Teams and Zoom, and other commonly used electronic programs;
  • Ability to establish and maintain effective working relationships with County officials, department heads, peers, associates, and the general public. 

Physical Requirements
This work requires the occasional exertion of up to 10 pounds of force and occasional exertion of up to 50 pounds of force; work regularly requires sitting, speaking, hearing and repetitive motions, frequently requires touching or feeling and occasionally requires standing, walking, climbing, balancing, reaching, pushing, pulling and lifting; work has standard vision requirements; work requires vocal communication to express or exchange ideas orally; work requires hearing to perceive information at normal spoken levels; work requires preparing and analyzing written or computer data; work is generally performed in a moderately noisy location (e.g. business office, light traffic.) Must be able to operate a vehicle and perform fieldwork. 
 
Work settings include the Department of Social Services, referral/resource agencies, and the homes of individuals. Home visits are made to often crowded, unsanitary dwellings, which lack proper lighting, handicap access, and other amenities. In addition, homes are often sub-standard and may be infested with pests. The worker is exposed to regular traffic hazards and adverse weather conditions.

Special Requirements
Must possess a valid North Carolina driver's license.
 
Must complete all applicable required training including: Confidentiality, Civil Rights, FEMA and Red Cross Shelter Training, Bloodborne Pathogens, applicable adult services program training, new employee orientation, and Chatham County Department of Social Services Policy and Procedures.

Salary : $49,583 - $63,215
