St. Margaret’s Center is a program of Catholic Charities of Los Angeles that serves as an access center for the L.A. County’s Coordinated Entry System (CES) in the northern part of the South Bay. St. Margaret’s Center is seeking a Client Resources Coordinator III (CRC III) that will work as part of the homeless services team. Under the direction of the Homeless Services Manager, the CRC III is responsible for providing individual case management to adults experiencing homelessness utilizing a theoretical framework incorporating Trauma Informed Care, Motivational Interviewing, Critical Time Intervention and Stages of Change.
The CRC III will perform specialized tasks including comprehensive case management services such as intake, assessment, goal setting, monitoring and reassessment, life skills, and referrals and linkages to other providers. The CRC III will ultimately serve as a housing navigator for people experiencing homelessness (PEH) to secure permanent housing through LA County’s Pathway Home program. The CRC III will document all participant interactions and input that data into the Homeless Management Information System (HMIS / Clarity).
Responsibilities include, but are not limited to :
- Manage a caseload of 20–25 households that have been matched to Housing Navigation slots.
- Rapidly enroll referrals received from the Los Angeles Homeless Services Authority (LAHSA) and / or interim housing providers, including gathering documentation needed for enrollment and creating Housing and Services Plans (HSP) in collaboration with participants.
- Assist participants including identifying and applying for housing units, time-limited subsidy programs, housing vouchers, and / or permanent supportive housing resources, based on individual goals.
- Collaborate with others to support the lease up process, including verifying property ownership, inspecting units, submitting move-in packets, and facilitating warm hand-offs.
- Connect participants to resources that support housing stabilization and promote wellness, including medical, mental health, substance use services, employment, etc.
- Engage households on a consistent basis to provide support related to HSP goals. Frequency of services will be based on the needs of the individual. Weekly visits are required for unhoused participants and home visits must occur at least once a month for housed participants.
- Provide trauma-informed services that align with Housing First and Harm Reduction philosophies, in accordance with agency and funder expectations, and abide by legal and ethical standards.
- Strive to recognize the best in each participant and support the meaningful change they seek through building relationships and utilizing motivational interviewing techniques and strength-based case management techniques.
- Develop professional relationships with housing providers, property owners, and property managers in order to navigate participants into permanent housing. Act as an advocate for participants who are experiencing housing barriers (e.g. poor credit, evictions, etc.).
- Ensure that landlords are respecting tenant rights and that participants are aware of the expectations and obligations outlined in their lease agreements.
- Connect participants to legal services when necessary to mitigate evictions or other tenant rights issues.
- Maintain client confidentiality and accurate and timely documentation in HMIS / Clarity, charting in accordance with agency policies and expectations.
- Complete annual recertifications, including assessing for ongoing eligibility.
- Explore opportunities for community integration to support housing retention in preparation for a successful termination of services when appropriate.
- Outreach and transport participants as needed to essential appointments and conducts visits within the community in Service Planning Area (SPA) 8 and Los Angeles County, including co-location at interim housing sites when assigned.
- Participate in supervision, staff meetings, and training, both internally and with partners agencies. Present participant cases at case conferencing meetings.
- Displays sensitivity to the participant population’s cultural and socioeconomic characteristics.
- Handle public contact and relations effectively.
- Plan short- and long-range activities.
- Other duties as assigned.
