Job Overview

Responsibilities

- Conduct assessments to identify participants' needs and tailor group content accordingly.
- Provide clinical counseling and support to group members, fostering an inclusive atmosphere.
- Utilize telehealth platforms when necessary to reach clients who may not be able to attend in-person sessions.
- Maintain accurate case management records and documentation of participant progress.
- Collaborate with other healthcare professionals within the clinic to ensure comprehensive care for clients.
