CHC is a non-profit social service agency in the Akron area whose mission is to treat, inspire, support and empower individuals and families impacted by the disease of addiction. CHC has been a critical part of Ohio’s efforts to treat and prevent substance use disorders since 1974.
We are currently looking for a Housing Case Manager for our OMCDC program. The Housing Case Manager will provide support to housing residents with emphasis on teaching residents how to remain in permanent housing.

We are currently looking for a Housing Case Manager for our OMCDC program. The Housing Case Manager will provide support to housing residents with emphasis on teaching residents how to remain in permanent housing.
Duties Include, but not limited to:
- Provide on-site supervision for the managed care group homes as required by the MCO.
- Provide weekly case management and life skills education to United Healthcare and other managed care housing members.
- Facilitate/assist with the move in for residents
- Meet with residents on a weekly/bi-weekly basis. (Meet in resident unit once per month)
- Maintain Case Management files with the required forms and documentation.
- Develop and implement a “life skills” curriculum.
- Enforce the housing leases and policies.
- A Bachelor’s degree or two years related experience.
- Basic computer and research skills.
- Familiar with community resources.
- Must maintain an insurable driving record and have a valid Ohio driver license.
