What are the responsibilities and job description for the Housing Advocate position at CHD Brand?
The Housing Advocate works with Persons Served by Adult Clinical Community Services (ACCS) to provide services in support of securing and maintaining permanent housing. The Housing Advocate also provides outreach services to a caseload. Housing Advocates work within CHD's team model, establishing open communication, support and accountability among co-workers.
Housing Advocates work with Persons Served and their Integrated Teams to:
- Gather housing history
- Develop a focused strategy for each Person Served as it relates to self - sufficiency, permanent housing and strengthening economic stability
- Access all available entitlements such as RAFT and Fuel Assistance
- Assist Persons Served in finding appropriate housing
- Provide tenant education regarding rental and landlord responsibilities
- Provide consistent open communication with landlords, building trusting relationships wot ensure longevity in permanent housing placements
- Recruit and develop strong and enduring relationships with landlords
- Negotiate leases and rent amounts
- Conduct outreach to landlords
- Support Persons Served to identify their housing needs
- Support Persons Served in budget preparation
- provide transportation of Persons Served to apartment walkthroughs and viewings
- Assist persons served with activities of daily living, e.g. food shopping and preparation, housekeeping, laundry and budgeting and banking
- Transports Persons Served in matters related to service delivery e.g. medical appointments, shopping, employment interviews, vocational programs, banking and laundry
- Deals with entry and termination issues
- provide crisis intervention
The ideal candidate will be a self starter with excellent written and verbal skills who can use a computer.
