What are the responsibilities and job description for the Licensed Practical Nurse position at CHD Brand?
Center for Human Development, (CHD), is currently is seeking a full-time LPN to work flexible business hours Monday-Friday, no on-call. The LPN would become part of a wonderful team of nurses that work with our behavioral health outpatient clinic in Springfield, MA. The LPN will oversee clinic triage, medication refill requests, lab review, prior authorizations, and collaborating with the client's care team, other medical providers, and other CHD programs.
WORK LOCATION
This position is an ON-SITE office position (M-F) with one day telecommuting available. The office is located in Springfield, MA
YOUR ROLE AS A LPN WITH CHD:
CLIENT SERVICES
- Develop strong, supportive relationships with clients
- Implementation of client medication treatment plans
- Interacting with clients in order to assess needs, provide education and information around health issues and the role of prescribed medications in their treatment
- Assistance in the management of medical issues
- Coordination of clients’ medical treatment with other service providers
PROGRAM AND STAFF DEVELOPMENT SERVICES
- Participation in team approach including communication with Program staff
- Ensuring adherence to established CHD policies and procedures for client treatment
- Consultation with Program staff for coordination of client treatment
- Attendance and Participation in weekly staff meetings
- Attendance and Participation in monthly meetings with the psychiatry team
MINIMUM QUALIFICATIONS
- LPN in good standing
- Two years of experience working with a psychiatric population preferred
- Valid driver’s license and registered vehicle
