What are the responsibilities and job description for the Licensed Practical Nurse position at CHD Brand?
The LPN provides ongoing Quality Assurance by monitoring a site’s medication administration system to ensure compliance with DCF regulations, licensing requirements and MAP policies (for MAP Programs) and CHD’s Medication Administration Program. This is accomplished by conducting reviews of medication administration practices at regular intervals. The LPN works within the agency’s team model, establishing open communication, support and accountability among co-workers. They are accountable to the Registered Nurse or Assistant Director of Residential Services and receives supervision from their immediate supervisor. The LPN provides direct oversight of the medication rooms for the regional programs. They ensure that policies and procedures are adhered to at all times and collaborate with the RN and program administration team to problem solve. They can provide in-service training to the staff as needed as well as provide any technical assistance. The assist in ensuring medications are reordered, medication reviews are scheduled/completed and all necessary paperwork is received from prescribers and they liaison between the program(s) and any external resources i.e. PCPs to make sure appointments are scheduled as needed/required
