What are the responsibilities and job description for the Outreach Caseworker Project Longevity Waterbury position at CHD Brand?
CHD is seeking an Outreach Caseworker to join our Project Longevity team in Waterbury, CT. Through this program, the Outreach Caseworker Project Longevity staff assists program participants to live independently in the community by addressing their housing needs.
YOUR ROLE AS A PROJECT LONGEVITY SPECIALIST:
As a Project Longevity Specialist, you will assist program participants to live independently in the community by addressing their mental and physical health, substance use, employment and housing needs. The Project Longevity Specialist actively promotes the philosophy of the agency and maintains a high level of commitment to program participants. Project Longevity staff use a counseling approach that uses both motivational interviewing and harm-reduction.
You will help the participants with:
- Using a Housing First approach to address housing instability, including eviction prevention and landlord mediation.
- Remove housing barriers and provide Rapid ReHousing support to help clients find affordable housing.
- Coordinate with landlords, complete paperwork, conduct home visits, and maintain documentation in HMIS and Veoci
- Monitor client progress, ensuring participation in best practice supports through regular communication with the Project Longevity Team
- Provide crisis intervention, assist with budgeting, and report any urgent issues to supervisory staff immediately
- Regularly assess client needs and connect them to appropriate services and providers
REQUIRMENTS:
- Experience working with psychiatric disorders or co-occurring population preferred
- Bachelor’s degree from an accredited educational institution preferred
- Ability to effectively manage crisis situations
- Must be comfortable engaging in the community for majority of the position
- Field visits within the surrounding communities to see participants
- High school diploma or GED required
- Reliable, registered & insured vehicle and approved driving record check
- Approved criminal background check (CORI)
Shift available:
- FULL TIME Monday-Friday 8:30am-4:30pm
