SUMMARY DESCRIPTION
The primary focus of this position is to coordinate the home-delivered meal service at assigned location(s). This includes determining client eligibility, creating meal schedules, and scheduling volunteers.

ESSENTIAL DUTIES & RESPONSIBILITIES
- Receive / intake referrals from Homebound Assistant.
- Home visit/telephone to complete client application and assess eligibility by utilizing CHEER and DSAAPD-approved questionnaires and forms.
a. Assess client needs for additional services and refer to appropriate departments/organizations. Follow up as needed.
b. Visit/contact homebound meal clients at least every six months to reassess eligibility.
c. Refer ineligible clients to CHEER congregate services or other nutritional / food assistance programs.
d. Utilize the ServTracker agency software program to document all demographics, case notes, assessments, referrals, and meal schedules.
- Conduct Nutrition Screening on an annual basis for all homebound meal clients and record in ServTracker.
- Increase client donations.
a. Request client donation/pledge at the time of application.
b. Responsible for monthly distribution of donation reminder letters.
c. Review the pledged amount with the client at reassessment.
- Creates, coordinates, and communicates meal schedules and changes with Center Directors, Kitchen Managers, and Volunteers.
a. Determine appropriate delivery route, sequence, and delivery days.
b. Update active/inactive status in ServTracker.
c. Record meal schedule suspensions in ServTracker.
d. Review delivered meal service units daily / weekly with the Center Director prior to the Center Director posting units.
- Responsible for, but working with Center Director(s), orienting and scheduling volunteers for homebound meal delivery.
a. Create/update Volunteer HDM Jobs Intake in ServTracker.
b. Maintain Volunteer calendar/scheduler in ServTracker with regular and substitute delivery persons.
c. Verify Volunteer calendar/scheduler accuracy to Homebound Assistant for accurate posting by Homebound Assistant of volunteer service units.
- Deliver meals as needed when Volunteers are not available.
- Attend Outreach Workers’ meetings, trainings, etc. as required.
- Mentor/train / cross-train/assist/support other Outreach Workers/coworkers in the Nutrition Program as needed.
- Maintain current listings of agencies, services, and organizations available to homebound meal clients and make appropriate referrals.
- Community outreach to increase awareness of CHEER services, and locate clients in need of service. May include personal meetings and appearances, phone calls, letters, flyers, or other methods as needed.
- Responsible for increasing the number of homebound meal clients served.
- Compile, prepare, and submit reports as required by the Program as needed.
- Responsible for maintaining assigned Agency vehicle including cleaning interior and exterior on a regular basis and conducting regular maintenance checks.
- Contributes to team effort by performing other related duties as assigned or required.

- Associate's Degree in Social Services and or related field or comparable experience in social work, geriatrics or counseling preferred.
- Able to multi-task in a fast-paced environment.
- Able to prioritize; excellent time management skills.
- Able to operate personal computer/printer. Experience with word processing and database programs is necessary.
- Able to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Able to write routine reports and correspondence.
- Ability to speak effectively before groups of customers/clients, agency employees, or service providers.
- Must maintain a Defensive Driving Certificate and keep a good driving record to operate an Agency vehicle. Must be able to travel to clients’ homes by car.
- Able to conduct interviews of a sensitive nature with compassion and patience and be able to maintain confidentiality.
- Other duties as deemed necessary by the Homebound Assistant (Supervisor)
REPORTING: Homebound Assistant
PRIMARY WORK LOCATION: Most duties and functions will be performed in an office environment although local travel to homebound clients’ homes and other CHEER locations is required to support programs and services as required in those locations. Occasional weekends and/or evenings may be required.
WORK HOURS: Monday - Friday: 8:00AM – 4:30 PM. Subject to change. Must be able to work occasional evening and weekend hours.
EMPLOYMENT STATUS: Full Time (40 hours/week) Non-Exempt.
