Position: Long-Term Substitute Child and Family Social Worker
Location: Chelmsford Pubic Schools
Assignment Dates: 01/06/2025- 03/14/2025 (Dates are subject to change)
Reports to: Director of Student Services and Coordinator of Social Emotional Learning & Counseling Services
Job Description:
The Social Worker will assist students with personal, social-emotional, psychological and environmental priority areas that affect a student's capacity to meet their school potential. The Social Worker will partner with students, families and caregivers, community agencies and school personnel to implement a collaborative approach to assess student needs and deliver programs and serves to maximize students' school success. The Social Worker will support children and families utilizing a clinical and case- management approach to address obstacles to school attendance, barriers to learning, and achieving their potential.
•Support the social, emotional and psychological well-being of students in order to reduce barriers to learning
•Observe, assess and provide services to students in a variety of settings (school, home, and community)
•Provide family support including parent support groups, home visits, parent trainings and educational workshops on adolescent
development, stress reduction and parenting strategies
•Build and maintain relationships with parents/families of students served, including phone calls, meetings, e-mail and when
needed, home visits.
•Communicate and collaborate with the School Counselors, Administrators, Supervisor of Students and School Resource Officer.
•Provide Crisis Intervention Services
•Provide referrals for outside services (counseling, support groups, etc.)
•Participate in meetings as necessary.
•Work directly with students as part of a multidisciplinary team
•Meet regularly with the Principal, Assistant Principal, Guidance Counselors, Psychologists and Nurses in order to coordinate
student services
•Collaborate with therapists, BCBAs and other direct service providers to support students and communicate with their teaching
team about their educational needs
* Develop plans to support students and families around increasing consistent student attendance
•Assess child mental health needs and implement short term, frequent and strategic interventions
•Facilitate groups for identified at-risk students to develop coping, social and decision-making skills
•Develop and provide professional learning opportunities to staff related to social emotional issues and how they impact learning
•Support school and districtwide social emotional learning initiatives, including curriculum in the areas of resiliency and student
mental health
•Assist with universal and targeted mental health screenings including (but not limited to): SBIRT, YRBS, RCADS
* Act as a liaison as required.
