Position Summary:
Chemik Hospice Care LLC is seeking a compassionate, nonjudgmental, and spiritually grounded Hospice Chaplain to provide emotional and spiritual support to patients and families facing terminal illness. As a vital member of the interdisciplinary team (IDT), the Chaplain helps ensure holistic care that honors the beliefs, values, and traditions of each individual, regardless of faith or background.
Key Responsibilities:
- Conduct initial spiritual assessments within five (5) days of admission and as needed.
- Provide ongoing spiritual support and counseling to patients and families based on their individual needs and preferences.
- Offer non-denominational support, respecting all belief systems and cultural backgrounds.
- Assist patients and families in addressing issues of meaning, grief, forgiveness, hope, and legacy.
- Participate in interdisciplinary group (IDG) meetings to contribute to the development and revision of individualized care plans.
- Provide or coordinate spiritual rituals, prayer, or sacraments when requested by patients or families.
- Offer bereavement support as part of the hospice bereavement program.
- Coordinate with community clergy or spiritual advisors as needed.
- Maintain timely and accurate documentation of all spiritual care services in the electronic health record (EHR).
- Provide support to hospice staff as needed to address emotional or spiritual stress related to caregiving.
Qualifications:
- Master’s degree in Divinity, Theology, Pastoral Counseling, or related field preferred.
- Ordination or endorsement by a recognized faith group or religious organization.
- Experience in hospice, palliative care, or healthcare setting preferred.
- Strong interpersonal, listening, and communication skills.
- Ability to provide interfaith and culturally sensitive spiritual care.
- Valid driver’s license and reliable transportation for patient visits.
