As a Clinical Coordination Specialist, you'll play a vital role in enhancing the quality of patient management and satisfaction. You'll promote continuity of care and cost-effectiveness through integrated functions of case management, utilization review, and discharge planning.
The Case Manager is responsible for conducting concurrent medical record reviews using specific indicators and criteria as approved by medical staff. This role requires a thorough knowledge of case management principles, healthcare management, and reimbursement.
Responsibilities:
- Coordinate the integration of social services/case management functions into the patient care, discharge, and home planning processes with other departments, external service organizations, agencies, and healthcare facilities.
- Act as a patient advocate: investigate and report adverse occurrences and perform staff education related to resource utilization, discharge planning, and psychosocial aspects of healthcare delivery.
- Promote effective and efficient utilization of clinical resources and mobilize resources to assist in achieving desired clinical outcomes within specific timeframes.
To excel in this role, you'll need strong written and verbal communication skills, presentation skills, and the ability to convey medical terms and treatment plans so they're understood by patients and their caregivers. A Bachelor's Degree in Nursing (BSN) or RN with a bachelor's degree in a related clinical field is preferred, along with a valid, active Registered Nurse (RN) license in the state of employment and a minimum of 2 years' clinical work experience.
