As a Clinical Medical Director, you will directly oversee and mentor primary care providers (PCPs) at your assigned center, ensuring excellence in clinical quality, efficiency, patient outcomes, and overall satisfaction. You will spend part of your time delivering direct patient care and the rest in crucial leadership activities, focusing on PCP performance, engagement, and fostering a strong culture in clinical operations.
Key Responsibilities :
- Provide high-quality care for older patients with acute and chronic conditions.
- Take accountability for patient care and outcomes while effectively collaborating with specialists as needed.
- Engage with hospitalists for patients under your supervision when hospitalized.
- Assess, diagnose, and manage complex health needs, providing education and care coordination.
- Lead a healthcare team, including medical assistants and care coordinators.
- Collaborate with transitional care teams for patients unable to visit the office.
- Facilitate weekly Super Huddle and Transforming Care Meetings, ensuring clinical leadership.
- Participate in center management, assist with patient recruitment, and support staff hiring.
- Mentor and support PCPs in achieving outstanding clinical outcomes.
- Work with center operations and financial performance to enhance center membership.
- Complete other duties as necessary.
