As a Clinical Operations Manager, you will lead and support primary care providers (PCPs) in a fast-growing clinical environment. Your responsibilities will include optimizing clinical quality, enhancing patient satisfaction, and ensuring outstanding clinical outcomes at your designated center. This position balances direct patient care with essential leadership duties, tailored to meet the needs of our team and community.
Key Responsibilities :
- Provide comprehensive care for patients with acute and chronic illnesses, focusing on older adult patients.
- Take accountability for patient care and outcomes while collaborating with specialists as necessary.
- Engage with hospitalists to coordinate the care of hospitalized patients.
- Lead and guide a dedicated care team, including medical assistants and coordinators, to enhance patient care.
- Coordinate care for patients unable to visit the office, working closely with transitional care teams.
- Conduct Super Huddles and Transforming Care Meetings to continuously improve clinical practices.
- Mentor and support PCPs to achieve excellent clinical outcomes.
- Collaborate with management to enhance center performance and patient recruitment.
- Perform additional duties as needed based on the demands of the center.
