Nurse Practitioner Family Practice
We are seeking a highly skilled and compassionate Nurse Practitioner to join our team in Riverdale, Georgia.
Job Description:
- Demonstrate accountability for patient outcomes, clinical care, and cost-effectiveness.
- Act as a health coach, helping patients set and achieve health goals.
- Simplify and prioritize care to facilitate behavior change.
The ideal candidate will be a strong leader, able to champion a positive culture of love, accountability, and passion. They will work collaboratively with other healthcare professionals, adhere to strict departmental goals, and provide high-quality patient care.
Responsibilities:
- Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with acute and chronic illnesses.
- Lead a care team, including medical assistants, care facilitators, and care coordinators.
- Engage with transitional care teams, case managers, and other resources as needed.
- Participate in risk and quality management programs, clinical meetings, and other meetings that promote patient health and company goals.
The successful candidate will possess excellent communication and leadership skills, a commitment to evidence-based medicine, and a passion for delivering high-quality patient care.
Requirements:
- ARNP or similar advanced degree in Nursing required.
- Current Nurse Practitioner Certification in the State of practice required.
- A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates.
