Job Description
We are seeking a Nurse Practitioner Family Practice to join our team at ChenMed in Riverdale, Georgia.
Responsibilities
- Provide primary care services to patients with acute and chronic illnesses
- Take accountability for patient outcomes and seek consultation from specialists when needed
- Engage with hospitalists and transitional care teams for hospital-bound and homebound patients
- Lead care teams and supervise Advanced Practice Practitioners (APPs)
Requirements
- ARNP or similar advanced degree in Nursing required
- Current Nurse Practitioner Certification in the State of practice required
- Board certification by AANP or ANCC preferred but may be required for certain States
- A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates
