Job Description
The Primary Care Physician will be responsible for providing high-quality medical care to our patients with acute and chronic illnesses encountered in the older adult population. The ideal candidate will have excellent communication skills, a strong work ethic, and a passion for delivering compassionate care.
Key Responsibilities
- Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with acute and complex chronic health needs
- Lead the care team consisting of care promoter, care facilitator, and care coordinator for patients able to come to the office
- Engage with hospitalist whenever one of their patients is in the hospital
Requirements
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty preferred
- Ability to obtain or already have a current, active State Medical License for the state(s) that are part of the Division in which he/she will be working
