Cherish Hospice is seeking a Registered Nurse Case Manager (RNCM) in the Springfield/ Dayton Area.
Responsibilities of an RN Case Manager-
- Lead the care team including LPN, Aide, Social Worker, Chaplain
- Assessing clients' physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans
- Working with clients, family and friend support networks and health care professionals to put care plans in place
- Listening to clients' concerns and providing counseling or intervention as required
- Recording clients' progress including charting referrals, home visits and other notable interactions
- Travel as necessary to service patients in their residence of care, which includes home, assisted living facilities, long term care facilities, and hospitals.
Qualifications for a Registered Nurse Case Manager (RNCM) Include, but are not limited to:
- RN License (Required)
- Driver's License (Required)
- BLS Certification (Preferred)
- Hospice or Home Health: 1 year (Preferred)
- Hospice and Palliative Medicine (Preferred)
