Cherish Hospice is seeking a PRN Dietitian.
PRN Dietitian Responsibilities :
Provide direct dietary assessment of patient.
Provide education to patient and caregivers regarding diet management.
Appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
Provide counseling and education to IDG, nursing staff and physician, as appropriate.
PRN Dietitian Qualifications :
Bachelor’s degree in nutrition or related subjects, preferred.
Current State Driver’s License.
Two years of experience as a Dietician in a clinical care setting, hospice or home health preferred.
Ability to establish a plan of care to meet the dietary needs of the patient.
