Case Manager/Community Health Coordinator
Monday to Friday 8:00am – 5:00pm (no weekends, nights, or major holidays)
Full-time, 40/hour per week
Pay starts at $18 an hour with bonus opportunities immediately after hire.
Locations available: Lenoir City, TN, In-Person

Key Responsibilities:
- Conduct assessments to evaluate patients’ needs, challenges, and goals.
- Develop personalized care plans or service plans tailored to patients’ specific needs.
- Provide referrals to appropriate community resources, including healthcare, mental health services, housing programs, and employment assistance.
- Offer emotional support and guidance to patients navigating difficult situations.
Qualifications:
- Bachelor's degree in Social Work (BSW), Psychology, Sociology, or a related field.
- Previous experience in case management, social services or direct patient support is preferred.
