Primary Function
The RN Care Manager performs a full range of nursing procedures independently, including assessing patients, developing and implementing care plans, administering treatment, and educating patients and families. The role ensures the physical and psychosocial well-being of patients of all ages by following standards of care and established policies.


Job Description

  • Utilize best practice models for panel management and establish health promotion strategies.
  • Address acute and chronic care needs through team-based management for panel patients.
  • Lead pod management to meet quality indicators and support Primary Care Providers.
  • Use iCare and care management platforms for documentation, tracking follow-ups, and ensuring care needs are addressed.
  • Coordinate specialty referrals and follow up on ED and ICC visits to provide timely PCP updates.
  • Develop and evaluate care plans and offer patient education on disease management, medications, and available resources.
  • Perform routine nursing care, specialty clinics, and, when needed, emergency care interventions.
  • Conduct assessments, collect data from patients and families, and develop patient-centered care plans based on these insights.
  • Function as charge nurse when assigned, overseeing staffing, anticipating potential issues, and leading problem resolution.
  • Initiate individualized care plans, addressing Social Determinants of Health (SDoH) and tailoring interventions for high-risk patients.
  • Execute nursing interventions, document care provided, and monitor medication administration and patient safety measures.
  • Provide patient and family education and support, coordinate with community resources, and ensure continuity of care.
  • Maintain safe practices, comply with OSHA regulations, and participate in performance improvement activities.
  • Respond to emergencies, perform resuscitation if required, and serve on committees as appointed.
  • May be required to adjust work hours, cover late clinics, and rotate between departments including inpatient, satellite clinics, and emergency.

Education, Licensure, Certification, and Experience

  • Bachelor’s or Associate Degree in Nursing from an accredited program.
  • Minimum of two years of nursing experience preferred.
  • Unrestricted NC RN license or equivalent through reciprocity.
  • Current Basic Life Support (BLS) certification, obtainable within 6 months if not already held.
  • Valid North Carolina driver’s license required.

Job Knowledge

  • Comprehensive understanding of nursing principles to meet the needs of patients from infancy to elderly.
  • Familiarity with disease processes, medications, and treatment interventions.
  • Proficiency in assessing patient needs, planning, and managing care, and interpreting clinical data.
  • Knowledge of community health resources, legal and ethical nursing aspects, and multi-disciplinary teamwork.

Complexity of Duties
The role requires flexibility, leadership within the pod, and adaptability to guide team members effectively. The RN Care Manager must make informed decisions independently and apply professional judgment in cases where guidelines do not apply, with oversight from the Outpatient Administrator.

Supervision Received
The RN operates independently, aligning patient care with established procedures, with the Outpatient Administrator providing oversight and evaluation.

Responsibility for Accuracy
The incumbent must follow policies strictly, as errors can impact patient health and recovery. Responsibility for accuracy largely rests with the RN Care Manager, with some verification by supervisors or other healthcare staff.

Contact with Others
Frequent, sensitive interaction with patients, families, and healthcare personnel, requiring professionalism, tact, and confidentiality.

Confidential Data
Handles highly sensitive patient information, adhering to confidentiality regulations as required by the Privacy Act of 1974.


Mental/Visual/Physical Environment
Primarily sedentary, with occasional requirements for walking, standing, lifting, and other physical activities to assist patients. Frequent interruptions necessitate strong prioritization skills.

Work Environment
Primarily in a clinical setting, with exposure to communicable diseases and other hazards, necessitating adherence to OSHA and health guidelines.

Customer Service
Consistently provides excellent customer service, aligning actions with CIHA’s core values, and proactively seeks to support patients and their families.
