Pending License accepted : (Y / N) : Yes; Must obtain prior to starting
RTO Restrictions : All RTO must be included on the initial submission. RTO requested after the fact will not be considered.

Schedule Information
REQ : 2 additional shifts per 6 weeks OR 2 on call shifts per 6 weeks
Unit Specific Information
MUST be able to pass arrhythmia and medication test on 1st day!

Attendance policy for travelers : If more than two shifts per 13-week contract are missed without reasonable explanation subject to SRHS discretion, contract will be reviewed for cancellation.
