Job Summary :

Provides care management to families caring for children and ensures their physical, social, emotional, cognitive, and behavioral health needs are met. Addresses challenges by providing individualized strength-based support, education, coaching, information, and resource referrals. Empowers families to work through challenges and access community resources.

Job Duties :

Routine duties shall include providing health care services to individuals eligible for services in accordance with the self-governance compact and funding agreement between the Cherokee Nation and the United States executed under the authority of the Indian Self-Determination and Education Assistance Act. Practices only within the boundaries of competence, based on documented education, training, supervised experience, state and national professional credentials, and appropriate professional experience. Advises individuals and families regarding issues including mental health, poverty, unemployment, substance abuse, physical abuse, rehabilitation, social adjustment, child care, or medical care. Interviews clients individually, in families, or groups, assessing their situations, capabilities, and problems, to determine what services are required to meet their needs. Conduct approved screenings and assessments to help determine the clients strengths and needs. Provides finds, or arranges for support services, such as child care, social services, or parenting classes, to help the client meet their needs. Serves as liaisons between students, homes, schools, family services, child guidance clinics, doctors, and other contacts, to help children and families work through challenges. Develops and reviews service plans in conjunction with families, and perform follow-ups assessing the quantity and quality of services provided. Provides support and counsels parents using an approved Parenting curriculum with child-rearing issues, interviewing the child (when appropriate) and family to determine whether further action is required for more intensive services. Refers clients to community resources for services such as mental / emotional health, job placement, debt counseling, legal aid, housing, medical treatment, or financial assistance, and provides concrete information, such as where to go and how to apply. Maintains case history records and prepares reports as required, and abides by HIPPA, departmental, and all other regulations. Collects supplementary information needed to assist clients, such as releases of information, medical records, or school reports. Consults with parents, teachers, and other school personnel to determine causes of problems such as truancy and misbehavior, and to develop and implement solutions with families. Conducts social research to keep informed of current child wellness and community resources. Responds to requests and referrals to assist families caring for children with challenges in meeting their physical, social, emotional, cognitive, and behavioral needs. Facilitates a process with families to identify strengths and needs. Clarifies and prioritizes the problem(s). Provides emotional support and identifies available resources, both formal and natural supports. Facilitates family access to resources (public and private sector) and coaches and supports families as they make calls to access resources. Provides guidance in completing forms and applications, and reviews documents to ensure family understands. Educates families regarding laws, policies, and procedures, and empowers families to articulate their concerns and needs effectively. Maintains local resource information for families to access independently and educates families in resource research and how to access resources independently. Advocates for families, as needed, to ensure proper services are delivered and the family is treated with respect, to the greatest extent possible. Attends staff meetings and scheduled required training. Supports the development and connection of families to natural supports within their community and assists with maintenance of the Resource Directory by updating information when appropriate. Maintains verifiable records at an institution or facility that has a published records retention policy that is equal to or exceeds the retention required by federal and / or state regulations. Maintains the confidentiality of any information received from any person or source about a client, unless authorized in writing by the client or required by law or court order. Obtains written informed consent of clients BEFORE audio / videotaping or recording or permitting third party observation of their activities. Obtains written consent from the joint-custodial parents, custodial parent, legal guardian or from the legal custodian appointed by the Court before conducting interviews of minor children. Complies with the applicable state, tribal, and federal regulations regarding the security, safety, and confidentiality of any behavioral health services record they create, maintain, transfer, or destroy, whether the record is written, taped, computerized, or stored in any other medium. Adheres to staff policies and procedures. Maintain HIPAA compliance and confidentiality at all times and protect all personal health information (PHI) as it relates to patient data. Other duties may be assigned.

This job has no supervisory responsibilities.

Qualifications :

EDUCATIONAL REQUIREMENT

Bachelor's degree from a 4-year college or university in Social Work or Social Science, Child and Family Sciences, Child Development, Organizational Leadership, Psychology, Community Health Education (not exercise or kinesiology), Elementary Education; or related field.

EXPERIENCE REQUIREMENTS

No additional experience.

COMPUTER SKILLS

An individual should know Spreadsheet and Word Processing software.

CERTIFICATES, LICENSES, REGISTRATIONS

Must possess a valid driver's license with a driving history verified through a motor vehicle report that meets requirements for Cherokee Nation underwriting rating.

OTHER SKILLS AND ABILITIES

Preference for speakers of Cherokee, Spanish, or American Sign Language.

OTHER QUALIFICATIONS

Must meet and maintain pre-employment and periodic background investigation and adjudication for child care. Educated on and compliant with HIPAA regulations; maintains strict confidentiality of client information. The employee must not be and will not be under sanction by the United States Department of Health and Human Services Office of the Inspector General (OIG) or by the General Services Administration (GSA) or listed on the OIGs Cumulative Sanction Report, or the GSAs List of Excluded Providers; pr listed on the OIGs List of Excluded Individuals / Entities (LEIE).

PHYSICAL DEMANDS

While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand; walk; sit; use hands to finger, handle, or feel and reach with hands and arms. The employee is occasionally required to stoop, kneel, crouch, or crawl and taste or smell. The employee must occasionally lift and / or move up to 10 pounds.
