What are the responsibilities and job description for the Patient Navigator position at Cherokee Nation?
ESSENTIAL DUTIES AND RESPONSIBILITIES
Advocates for the patient in understanding their healthcare, insurance, and communication with care providers.
- Coordinates treatment with healthcare teams and collaborates with providers.
- Engages in outreach initiatives to promote clinic/patient relations.
- Collaborates with partners and external entities to increase access to health care and other resources.
LEVEL OF RESPONSIBILITY
Performs work under direct supervision.
- Handles basic issues and problems, and refers more complex issues to higher-level staff.
- Possesses beginning to working knowledge of subject matter.
QUALIFICATIONS
Associate’s degree from a 2-year college or technical school, or equivalent combination of education and experience.
- No additional experience.

PHYSICAL REQUIREMENTS
While performing the duties of this job, the employee is regularly required to sit and talk or hear. The employee is occasionally required to stand; walk; use hands to finger, handle, or feel and reach with hands and arms. The employee must occasionally lift and/or move up to 25 pounds.
CERTIFICATES, LICENSES, REGISTRATIONS
Must possess a valid driver’s license with a driving history verified through a motor vehicle report that meets requirements for Cherokee Nation underwriting rating.
