JOB DUTIES AND RESPONSIBILITIES OF A RESIDENT CARE COORDINATOR
The responsibilities of the Resident Care Coordinator include, but are not limited to:
- Work with resident administrative services liaison to develop, execute and maintain resident care plans that meet the individual needs of the resident and the physician recommendations.
- Work with the nurse, med-aide and caregivers to build relationships and strong communication with residents, families and associates.
- Work with administrator and nurse to maintain a documented system of operations policies and procedures and ensure that all facility practices and policies are carried out in the highest ethical manner. Make recommendations for new or changes in procedures, policies, methods, education, reference material and general resident care and nursing practices to ensure the highest level of quality patient care is given.
- Assist management with the human resource function, including the design of an organizational structure adequate for achieving the facility's goals and objectives, recruitment, training, performance evaluation of staff.
- Help oversee the following day-to-day operational functions:
- Oversee all direct care staff (caregivers, med-aides) and ensure they are performing duties as required by Resident Service Plans
- Handle direct care staff scheduling and ensure all shifts are adequately covered.
- Audit medication administration, ensuring adequate ordering, receiving, documentation, and safe storage of physician-prescribed medications for residents. Conducts regular review of medication documentation and survey compliance.
- Make daily rounds to ensure that all direct care staff are performing their work assignments.
- Order and track inventory of various supplies, including equipment, diabetic supplies, and incontinence supplies
- Train new care staff and med-aides on caregiving, med administration and passing, campus software and computer programs, and facility procedures and practices.
- Arrange staffing to best cover the floor based on residents’ needs; rearrange as necessary and communicate changes and updates to all parties.
- Work the floor and fill in as needed.
- Audit medication rooms weekly to ensure order and compliance.
- Know and thoroughly understand each position (caregiver and med-aide) to enhance working relationships, functionality of the community, harmony, residents and resident care, and the overall business model.
- Lead by example with a positive attitude, appropriate workplace behavior and language, and a willingness to model and perform what you ask of others.
- Assist in the recruitment and selection of staff.
- Work with and seek counsel from HR for proper documentation, serious violations, performance or behavioral concerns, and terminations.
Job Qualifications:
- Medical Assistant (Certified preferred) or Certified Med Aide, CNA, or CNA-2.
- Three years’ experience in healthcare setting with a minimum one year as Med Aide, RCC, or care staff.
- One year of experience supervisory or lead position.
- One year of experience managing scheduling/staffing with full understanding of coverage needs.
- Knowledge of OARs for an ALF/RCF or related licensure.
- Experience and knowledge of the State survey process.
- Experience with Electronic Medical Records and computer documentation systems;
- Working knowledge of Microsoft 365 office technology and software (Office apps: Office: Word, Excel, Outlook, etc.)
- Excellent verbal communication, good interpersonal skills, and leadership qualities.
- Ability to professionally and diplomatically communicate with outside agencies, and family members.
- Excellent written communication with ability to professionally correspond via email and/or letter using proper documentation, grammar, spelling, and punctuation.
- Ability to make good sound decisions in stressful situations or when put on the spot.
