Position Summary
The Clinical Pharmacist is a professional healthcare employee who performs specific job responsibilities, governed by the Nebraska DHHS Pharmacy Statutes, for the Cherry County Hospital and Clinic. An individual who is responsible for the day to day activities in the pharmacy; who reports to the Pharmacy Director, and is accountable to the same. An individual with the duty of providing pharmaceutical services to all hospital services and disciplines, working as an integral part of the multi-disciplinary team to provide the best health care to our patients.
Duties and Responsibilities
Primary Responsibilities include but are not limited to the following:
- Dispensing medications as prescribed by a suitably licensed professional. All dispensing functions are the responsibility of the pharmacist. If done by a technician or intern, the work must be checked and verified by a pharmacist
- Compounding and preparing ordered pharmaceutical preparations, including intravenous additive solutions, and chemotherapy
- Cleans and disinfects sterile product (IV) preparation rooms
- Supervising all clinical functions of pharmacy interns and technicians, as required by regulation
- Preparing TPN solutions. Aiding in the monitoring of total parental nutrition (TPN) patients as required.
- Monitoring patients under the Antimicrobial Stewardship program
- Monitoring patient medications with respect to dosing, for renal or hepatic adjustments
- Fielding medication inquiries from staff members including nursing and providers
- Consulting with physician and nursing staff concerning availability, form, side effects, incompatibilities, therapeutic substitution, and hazards of currently available drugs
- Contacting and conferring with physicians if there are any questions, or irregularities regarding medication orders
- Completing medication reconciliation with patients, with counseling as necessary; and maintaining patient profiles in the chart as appropriate
- Maintaining inventory and records of narcotics. Investigating, and documenting on any discrepancies of controlled medications (C2, C3-5), and reporting to the Pharmacy Director
- Watching for any signs of potential medication diversion, and reporting to the Pharmacy Director
- Recording prescription information in all necessary databases, including the Nebraska Prescription Drug Monitoring Program, as required
- Maintaining departmental records as required.
- Packages medications into U/D containers, including bulk oral liquids
- Enters medication orders into computerized system, from written or verbal orders, or per verified facility protocols verified annually by the Medical Staff
- Building medications, order strings, and updating drug dictionaries as needed in the electronic health record.
- Submitting, reviewing, and completing computer related tasks and training as appropriate. Making computer entries as required.
- Building and maintaining medications and rates for the medication pumps
- Organizing daily operational duties. Pharmacy interns, technicians, and clerical personnel are directed by a Pharmacist.
- Completing and auditing patient charges and conferring with financial staff, as needed, to ensure accurate billing for patients
- Assuming purchasing and inventory control responsibility under established policies, as set out by the Pharmacy Director.
- Call responsibilities as assigned
- Developing and presenting in service training programs for other institutional professionals.
- Developing, reviewing, and revising medication policies and procedures as assigned by the Pharmacy Director
- Serving on committees or attending meetings as necessary (may include P&T committee, Antimicrobial Stewardship committee, Pharmacy/Nursing meetings, etc.)
- Attends and contributes to department staff meetings and in service programs as assigned
- Maintaining open communication with the Pharmacy Director, with pharmacy staff, and with other departments; as necessary, in the course of normal pharmacy operations.
- Establishing goals in measurable terms; evaluating and revising goal projection with a supervisor during scheduled evaluations.
- Instructing, evaluating, and initiating goal setting with technicians and clerical personnel, as directed by a supervisor.
- Alerts supervisor to accident producing hazards and promotes safety; knowledgeable of fire and evacuation protocols
- Makes economical use of time, supplies, and equipment
- Maintains an organized and hygienic working area
- Making recommendations to the Pharmacy Director regarding any necessary equipment or other improvements for daily operations
- Respects and preserves patient confidential information. Ensures the confidentiality of patients’ medical, personal, and financial records is maintained
- Knowledgeable of the Quality Assurance projects in the department, and active in quality improvement
- Knowledgeable of Pharmacy Department policies and procedures, and is in compliance with them
- Remains current and up to date on pharmacy laws, and regulations, and practices in compliance with them
- Knowledgeable of and committed to practicing Corporate Compliance policies and procedures
- Complies with Cherry County Hospital personnel policies
- Maintains compliance and familiarity with all pertinent hospital policies and procedures
- Performs other duties as assigned by Director of Pharmacy
Additional Responsibilities may include but are not limited to the following:
- Receives medication inventory orders, and updates system as necessary
- Restocks pharmacy shelves and supplies, as needed
- Participating in the annual inventory as needed
- Updates medication price changes; assists other departments with medication charging information
- Prepares medication daily fill, as needed
- Checks medication outdates, as needed
- Checks and refills automated medication systems and various floor stocks, as needed
- Reviewing and maintaining medication outdates in the pharmacy, ambulance, pyxis, and any medication boxes (including the crash carts), or floor stock; either directly, or with the help of other appropriate staff.
- Reviewing automated medication system reports, and editing any counts or configurations as needed

- Must be knowledgeable of state and federal confidentiality laws, including but not limited to HIPAA, and be familiar with and follow all policies, procedures and instructions regarding the privacy and security of protected health information applicable to the position.
Physical Demands and Working Conditions
- Physical mobility to be able to restock supplies and medications on existing shelving
- Ability to use available protective equipment to protect against potentially toxic substances/infectious disease

Requirements
Minimum Job Requirements
- Eighteen (18) years of age or older
- Bachelor's Degree in Pharmacy or Hospital Pharmacy Administration or PharmD
- Must meet State requirements for Pharmacist by the Nebraska Board of Pharmacy
- Minimum of three years experience in hospital pharmacy preferred, but not required; with minimum one year experience in hospital pharmacy highly preferred
- Must not have ever been convicted of any drug-related misdemeanor or felony
- Ability to work with other health care professionals
