Program Overview:
For individuals transitioning from closed custody correctional facilities, Cherry Health provides Person Centered Reentry Services to ease the transition back into the community. Services are available at two locations: our Community Treatment Center (CTC) in Detroit and Community Alternatives Program (CAP) in Grand Rapids. Services include 24-hour residential treatment in a structured environment with individual and group counseling services. Some participants come to us directly from a correctional facility, while others come as part of a diversion program to keep them out of prison. Together, we work to address health issues (including behavioral health issues) and other reentry needs i.e., employment, education, housing, and fostering connections to help justice system involved persons successfully reintegrate back into their respective communities.
Responsible for coordination and clinical supervision of services and case management including treatment plan development, ancillary services, case reviews, group counseling, aftercare planning, and client case monitoring. Supervision and professional development of the clinical team.

Requirements:
Bachelor’s degree required.
Current valid driver’s license required.

One year of experience in guidance, counseling, or job placement is required. Experience in a correctional-type residential program is preferred.
Responsibilities:
- Meet with each resident upon admission to complete Job Readiness Screening.
- Provide employment training to residents, including resume development as needed.
- Work closely with residents and community to obtain resources for prospective employment.
- Complete all initial contacts with employers as regulated by the FBOP Statement of Work and provide information concerning the Residential Reentry Center and FBOP accountability requirements.
- Conduct initial and on-going job site visits in conjunction with the On-Site Employment/Home Inspector.
- Coordinate ongoing communication with employers for feedback and follow-up regarding resident employees; trouble shoot problems residents may have with employers.
- Monitor resident’s movement in relation to work and spot check attendance at work.
- Review employment verification forms and confirm accuracy with employer via telephone.
- Generate work passes to employed residents.
- Keep track of 60-hour limit.
- Facilitate at least bi-weekly contact with Case Managers regarding a resident’s employment status: employed or unemployed.
- Complete all required documentation in an accurate and timely manner.
- Maintain accurate employment records in residents’ files; collect accurate information regarding subsistence payments and coordinate delinquent accounts with case managers.
- Attend FBOP conferences, trainings and workshops as directed.
- Other duties as assigned.
