Overview Of Position
Responsible for coordinating nursing aspects of perinatal care to prenatal patients and providing education, home visits, case management, care plan implementation, and management of Maternal Infant services to the prenatal, postnatal, newborn infant, and family. Must adhere to the organizational Policies, Procedures, Code of Ethics, and all licensing and accreditation standards of the position. Must represent the organization’s Mission in all actions and communications.
Essential Duties And Responsibilities
- Provide education, care coordination, care plan implementation, and management of Maternal Infant Health Program (MIHP) services to prenatal and postnatal women and infants up to one year. Services are primarily provided in the home.
- Work directly with medical providers in providing prenatal care and in developing and implementing a case management plan for each pregnant woman.
- Participate in interdisciplinary planning with social workers, dietitians, medical providers, and community health workers for meeting patient needs and health service objectives.
- Provide teaching and support on family planning, pregnancy, chronic disease, infant health and development, and other medical issues as needed.
- Complete MIHP risk screening assessment on new patients, progress notes on all visits, and discharge summaries upon completion of MIHP services within time requirements.
- Perform initial intake visits on prenatal and infant patients. Provide appropriate referrals to address the needs of the prenatal and/or infant patient.
- Complete medical intake for new OB patients entering prenatal care.
- Provide follow-up for no-shows or poor compliance patients.
- Participate in quality assurance, including chart audits and caseload reporting.
- Perform other duties as assigned.

- Current Michigan license to practice as a registered nurse in the State of Michigan, BSN preferred.
- Basic Life Support/CPR training according to requirements of license and/or certification.
- Valid Michigan driver’s license.

- Frequent telephone and computer use, requiring manual dexterity for writing and computer use.

- Specific vision abilities required by this job may include close vision, distance vision, and the ability to adjust focus.
- May require bending, squatting, or kneeling and lifting up to 50 pounds.
- Use of Personal Protective Equipment (PPE) as required.
