Cherry Health is a Federally Qualified Health Center (FQHC) with a mission to improve the health and wellness of individuals by providing comprehensive primary and behavioral health care while encouraging access by those who are underserved. The Social Worker / Behavioral Health Consultant (BHC) is a member of the patient's care team, providing brief action focused interventions to address a wide range of physical health, mental health, and substance use disorders to clients in a fast-paced setting. Primary functions of the BHC include identifying and treating co-occurring physical and behavioral health needs in the medical setting : screening, assessments, diagnostic interviewing, coping skills development, and care coordination with internal and external resources, to improve outcomes and decrease barriers for patients accessing healthcare services. This role is integrated into the Barry Community Health Center in Hastings, Mi.
