Purpose: The Community Resources Navigator promotes health and wellness of individuals whose needs extend beyond what other CSMM programs provide, such as extensive physical and/or mental health challenges.
This role is also responsible for assisting with infectious disease response and coordination, including but not limited to COVID.

Essential Functions:
- Connect individuals with appropriate community resources and provide follow-up support in effort to reduce barriers
- Identify effective placement opportunities for High Acuity individuals out of the scope of care for Cherry Street services
- Coordinate transportation and oversight of Isolation/Quarantine Site for guests who test positive for COVID or other infectious diseases
- Responsible for managing case load in an effective and efficient manner, completing documentation, and meeting requirements outlined in all Standard Operating Procedures
- Conduct crisis intervention and debriefing, including but not limited to verbal de-escalation
- Apply the understanding of stages of change and technique of motivational interviewing
- Perform risk assessment and implement harm reduction techniques both onsite and when conducting home visits, including but not limited to safety of self and others
- Seek to advance skill set needed by attending internal and external training opportunities and attend all mandatory staff meetings and trainings
- Responsible for incident reporting and responding to emergencies accordingly
- Supports organization by upholding mission, vision, and values of Cherry Street Mission Ministries in behavior and speech
- Essential Functions may be revised upon management discretion
Qualifications
Education and Experience
- High School Diploma or equivalent required Preferred, Associate's Degree in Social Work, Counseling, and/or other related field such as Peer Support or Chemical Dependency Counseling Assistant (CDCA) or at least (2) years experience in mental health field, shelter care, or similar

Licenses, Certificates, and Other Requirements
- Driver's License Required
- Mental Health First Aid Certified or completed within the first year of employment
- CPR Certified or completed within the first year of employment
- Suicide Prevention Training or completed within the first year of employment
- Trauma- Informed Care Training or completed within the first year of employment

Lifting Requirements
- Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
