POSITION SUMMARY
The Rehabilitation Specialist is responsible for conducting and developing clients’ rehabilitation assessments, providing support to PRP specialists, and reviewing PRP progress notes for completion. The Rehabilitation Specialist assists in developing and reviewing individualized rehabilitation plans to meet the needs of each consumer in accordance with State regulations. This includes implementation and documentation as directed by COMAR. Other duties as assigned to provide support directly to the PRP Director. This position splits time between PRP specialist duties and Rehabilitation Specialist duties and reports to the PRP Director.

PRIMARY TASKS AND DUTIES
- Participate in staff meetings as required/needed.
- Educate PRP Workers on how to provide rehabilitation services to correlate to the goals identified in the individual’s IRP.
- Screen referrals, schedule and complete rehabilitation assessments. Review and coordinate the collection of relevant information on referred client's background, social history and past treatment interventions in order to develop rehabilitation plans for clients.
- Provide quality review of notes and monthly summaries, ensuring goals work listed IRP’s
- Assure notes are being completed in a manner consistent with Federal and State statutes, rules, and policies and procedures, respectively.
- Rehab Specialist will carry a caseload around 15 clients and complete at minimum 3 visits per client each month.
- Rehab Specialist will complete discharge summaries for clients within 15 days in the Electronic Health Record.
- Rehab Specialist will obtain authorization for PRP services for clients through Provider Connect.
· Utilize the agency’s electronic health record (EHR) to document all client services and progress.
· Other duties as assigned.
ESSENTIAL FUNCTIONS/KEY COMPETENCIES
- Demonstrate ability to utilize computer skills to complete necessary
documentation in Insync (EHR) and use other office equipment
effectively.
- Able to communicate with staff and patients effectively.
- Knowledgeable of outside community resources for clients and families.
Experience
- Master’s Degree with LMSW licensure OR
- Psychiatric Rehabilitation Certification for Children and Adult from the Psychiatric Rehabilitation Association
1 Year experience in social work or case management field preferred.
