What are the responsibilities and job description for the RN Medicare Wellness Nurse position at Chesapeake Health Care?
POSITION SUMMARY
The Medicare Wellness Nurse will ensure that regularly scheduled preventative care is provided to our Medicare patients that they are entitled to in an effort to improve their overall health functioning. The Medicare Wellness Nurse will schedule and perform Medicare wellness appointments for our patients. They will collaborate with the health care team to ensure the highest level of service is provided. This position will report to the Chief Nursing Officer.
PRIMARY ACCOUNTABILITIES
Achieve Results
- Improve Medicare patient outcomes and deliver quality care within practice areas as assigned. Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, and patient satisfaction.
- Ensures timely and accurate collection of patient data, and makes observations on assigned patients to provide supportive data in the patient assessment.
- Promotes attainment of specified goals through health education to patients/families, and health care personnel as delegated by the registered nurse.
- Ensure all patient charts and related documentation are maintained current and relevant at all time.

- Ensure all care and nursing practices are conducted consistent within current, relevant standards, as well as state and local laws and regulations, The Joint Commission accreditation and related professional standards.
- Demonstrate competency in practice and knowledge of current standards of practice. Maintains credentials and performs current nursing practice expectations within established guidelines.
- Provides care in a manner that respects the patient’s rights and choices in a multi-cultural setting.
Relationships
- Develops and maintains favorable internal relationships, partnerships with co-workers, including clinic managers, nursing staff, physicians and business office staff.

PRIMARY TASKS & DUTIES
- Provides annual Medicare wellness visits for HHC patients.
- Assist in pre-visit planning and identify patients who may benefit from an
integrated visit with care management/coordination services.
- Is familiar with Medicare requirements and is able to perform within a given scope of practice.
- Will work collaboratively with the care management team and refers patients for care coordination and health coaching when appropriate.
- In collaboration with patient and healthcare team, establishes individualized, objective action steps that are time specific to assist the patient in improving their overall health.
- Provides ongoing support and evaluation of progress toward stated health goals.
- Revises the healthcare plan as needed in collaboration with patient and
- healthcare team.
- Acts as an advocate for patient and helps them to obtain their maximum level of health. Provides education to patients about their specific conditions and needs.
- Involves patient’s family, community, and other natural supports when authorized by patient in an attempt to promote a maximum level of independence.
- Determines the need for and completes appropriate referral processes.
- Maintains EHR documentation and data. Completes all documentation in a timely manner and accurate billing from the provider.
- Participates in regular team and organizational meetings. Participates on
- committees as applicable. Assists in the orientation of new personnel.
- Precepts and mentors other staff members. Promotes a collaborative atmosphere across the organization.
- Participates in Patient Centered Medical Home team meetings and quality
- improvement initiatives.
- Collaborates with the RN Quality Compliance Manager in improving the
- policy/procedures for the Care Coordination program.
- Provides educational opportunities and is a resource person for providers and staff.
- Meets with supervisor regularly to provide patient and staff updates, identify issues, and develop strategies for resolution.
- Develops and maintains relationships across organization. Displays compassion and skill in communication, understanding, and motivating others to reach their highest potential.

POSITION REQUIREMENTS
Education
- Graduation from an accredited nursing program
- Bachelor’s degree preferred
- Current, relevant state licensure as a Registered Nurse. New graduates must complete the state board examination within 90 days of graduation.
- BLS required
Experience
- Demonstrated success in customer service or working with the general public, preferably in a medical care facility.
- Significant knowledge of medical practices and procedures in a primary care environment.
- Demonstrated success and experience with direct patient services (e.g., phlebotomy) and the proper knowledge and use of medical devices and equipment.
- Familiarity with Patient Centered Medical Home model’s purpose and goals.
- Acknowledge patients’ rights by maintaining HIPAA guidelines at all times.
Physical/ Environmental
- Occasional lifting of 40 – 50 lbs and pushing of 5-20 lbs
- Sitting, standing and walking approximately 50% of the time
- A medium to high level of manual dexterity required
- Bending and reaching approximately 20% of the time
- Normal accessibility and mobility throughout the region required
- Normal overtime/extended work hours
Job Type: Full-time
Pay: $32.00 - $45.00 per hour
