Requirements:
- High school diploma or equivalent
- Current COVID-19 and flu vaccines required
- A valid driver's license
- Will be required to utilize your own car
- Some experience in health care and/or community engagement or previous training in a CHW course a plus
- CHESPENN WILL PROVIDE TRAINING - NO EXPERIENCE NECESSARY if requirements are met and interested in learning
- Current or former resident of the service community strongly preferred; experience in community/outpatient settings a plus
- Genuine passion and desire to enter a helping profession in health care or social services
- Ability to successfully complete a Community Health Worker curriculum
- Bilingual/bicultural fluency a plus
- Help patients develop health management plans and goals
- Coach and coordinate patients in the management of their chronic health conditions and self-care
- Establish positive, supportive relationships with participants and provide feedback
- Facilitate communication and client empowerment in interactions with health care/social service systems
- Facilitate communication and coordinate services between providers
- Work effectively with stakeholders (patients, staff, partner agency representatives, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions
- Continuously expand knowledge and understanding of community resources, services and programs provided
- Identify and apply appropriate role definition and boundaries
