Minimum Qualifications

Who We Are :

Mental Health Support Services (MHSS) is the provider of mental health, developmental disabilities, and substance use disorder services for children and adults in Chesterfield County. The Psychiatric Rehabilitation Services (PRS) program is currently seeking a Housing Case Manager to work with program participants with Serious Mental Illness (SMI) or dually diagnosed SMI and Substance Use Disorders (SUD), and potentially exiting homelessness, to locate community housing, apply for and obtaining a lease, and providing support to maintain positive tenancy upon move-in.

Minimum Qualifications :

Bachelor's degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field; two years of experience in a human services field; or an equivalent combination of training and experience. Two (2) years of experience working with adults with serious mental illness or dually diagnosed SMI / SUD with a Community Services Board and / or community settings preferred. Experience with housing placement and stabilization preferred. Experience working with people experiencing homelessness preferred. Familiarity with area homeless services organizations preferred. Experience using an electronic health records system preferred. DLA 20 or SOAR Certifications preferred. Bilingual Spanish preferred.

Required Knowledge, Skills, and Abilities :

Working knowledge of case management methods, practices, and procedures. Ability to interview clients to identify deficiencies in personal, social, economic and health needs; to work in a multi-disciplinary team environment providing treatment and rehabilitation services; to communicate effectively orally and in writing; to operate a personal computer and related software and other standard office equipment; to develop and maintain effective working relationships with internal and external customers. Knowledge of Landlord-Tenant and Fair Housing law and / or Housing First principles preferred.

Additional Requirements :

Current valid driver's license and good driving record required. Based on the Virginia DMV point system, records must not reflect a total of six or more demerits within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months. Out of state driving records must be obtained by applicant and presented at time of interview. Records must reflect at least three years of history and be dated within thirty days of interview date.

Pre-employment drug testing, FBI criminal background check, and education / degree verification required.

Duties

Duties include but are not limited to the following :

  • Assists individuals with identifying and applying for affordable housing options;
  • Identifies appropriate housing units in our community that meet all supportive housing standards including quality, privacy, integration, accessibility, and affordability;
  • Travels to various sites in the community for the purpose of monitoring housing stability and progress toward supportive housing plan goals; transports individuals to ensure informed choice in housing; connects to and monitors community services for children and adults with intellectual and developmental disabilities;
  • Assist individuals with the development and maintenance of a housing support plan; collaborates with community partners for advocacy for individuals with disabilities;
  • Provides guidance to tenants and clinical staff in tenant-landlord and fair housing laws, including the use of reasonable accommodations;
  • Educates clients and their families on their legal rights and provides information on department policies, procedures, and services;
  • Prepares and maintains casework documentation as required in the delivery of services (e.g. case histories, assessments, treatment plans, progress notes, correspondence); documents direct services and collateral contacts in the electronic medical record system;
  • Performs other related duties as required.
