Case Manager
CRCSI is hiring a Full Time Case Manager for our Assertive Community Treatment (ACT) Program.
What we do here:
The Assertive Community Team (ACT) is a full support service program incorporating case management and treatment for any of your loved ones experiencing severe and persistent mental illness, such as Schizophrenia, Bipolar Disorder, etc.
As a primary, direct service, ACT provides comprehensive and intensive outpatient mental health treatment as well as rehabilitative and support services to anyone who needs help beyond traditional community mental health services.
How does this job contribute to the program?
The employee will be expected to function as a caseworker as part of the therapeutic team for Assertive Community Treatment Team (ACT) consumers and to ensure maintenance of standards for compliance with regulations and licensure. The incumbent will work closely with the ACT treatment team and psychiatrist in formulating and executing treatment and transportation needs that conform to current standards of care.
The employee will be responsible for all aspects of care delivered in an ACT program to include psychiatric rehabilitation, community integration, housing, employment, and service coordination.
Duties and Responsibilities:
- Participate in daily team meetings, treatment team meetings, meetings with family or other providers as needed regarding the treatment needs of the consumers of ACT and collaborates with outside agencies, healthcare partners, family, and others in a professional and organized manner.
- Participates in the duties associated with engagement, admissions and discharges, plans and facilitates skill teaching groups and therapeutic activity groups and provides scheduled individual and group counseling.
- Develop a therapeutic relationship that will enhance teamwork, quality of program, and consistency across the program with other ACT staff members.
- Work with other agencies and service providers to access necessary community support for ACT consumers and maintain contact with other agencies involved in the care of ACT consumers, so as to promote continuity of care.

- Provide individual and group skill teachings and supportive counseling to consumers receiving ACT services in accordance with their treatment plan, as appropriate for the diagnosis, and following best practice standards and protocols.
- Conducts assessments and monitors consumers’ progress, both formally and informally for the purpose of providing quality treatment and tracking outcomes. Continuously evaluates the status of clients, ensuring that appropriate planning and coordination of treatment activities are in place to meet immediate needs
- Provide case management; psychiatric rehabilitation-based skill teachings, assistance with daily living skills, transportation, medication delivery, and any appropriate therapeutic or coordination services necessary.
- Provides appropriate crisis intervention.
- Develop and implement treatment plans and treatment plan reviews in accordance with program standards and fidelity.
- Will rotate on a scheduled “on-call” basis as necessitated by the program needs and staffing needs.
- Performs independently other duties as assigned.
Essential Job Functions:
- Must be able to appropriately and safely operate a motor vehicle.
- Must have and continue to have a valid driver’s license.
- Must be able to consistently pass a motor vehicle record background check with frequency of checks being determined by the agency.
- Demonstration of sound clinical judgement and logical problem-solving skills.
- Ability to professionally relate and effectively communicate information verbally and in writing to all internal and external customers, including consumers, CRCSI staff, and staff of other organizations.
- Knowledge and appropriate use of internal and external resources in the community.
- Proficient in basic computer skills, as well as competent documentation skills in the use of CRCSI electronic medical record.
- Possess willingness to learn, ability to process information and effectively adapt to change quickly.
- Ability to work varying hours and days and be flexible with scheduling changes as dictated by program and staffing needs.
- Extended sitting and driving.
- Extended sitting and completing tasks using telephone and computer.
- Ability to function as a part of a team.
- Maintain appropriate attendance standards.
- Maintains appropriate level of productivity as set forth by ACT program manager.
- Completes required documentation in a timely and legible manner, consistent with appropriate documentation, clinical and agency standards.
- Maintain the ability to rotate on a scheduled “on-call” basis as necessitated by the program needs and staffing needs
- Participate in ongoing clinical development, trainings, and supervision.
What is a typical schedule for this position?
This position will work Monday to Friday 8:30 am to 4:30 pm. On-call hours may be required as needed for client care.

Qualifications
Education: Bachelor’s degree in a human service field.
Experience: One year mental health experience preferred
Other Requirements: PA State Criminal, Child Abuse Certification, and FBI Clearances and a Valid Driver’s License with a Motor Vehicle Record that meets acceptable standards of CRCSI Auto Insurance Carrier.
