Job Description: As a DSP, you will provide essential support to individuals with mental health conditions, helping them achieve their personal goals and enhance their quality of life. You will work closely with individuals to promote independence, community integration, and overall well-being.
Key Responsibilities:
- Assist individuals with daily living activities, including personal care, meal preparation, and household tasks.
- Support individuals in participating in community activities and social events.
- Implement individualized care plans and behavior support plans.
- Monitor and document individuals progress and report any changes to the supervisory team.
- Foster a safe and supportive environment for individuals.
- Provide transportation to appointments, activities, and other outings as needed.
- Collaborate with other team members to ensure comprehensive care and support.

Qualifications:
