Chestnut Ridge Nursing and Rehabiliation is seeking a caring and passionate full-time Social Worker to join our Social Services Team.
Responsibilites:
- Identifying individual social and emotional needs
- Developing and Maintaining individualized care plans
- Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
Full Time: ( Monday-Friday 9am-5:30pm)
Requirements:
- Previous experience working in long-term care is preferred
- Bachelors degree in Social Work, Sociology, Psychology, or Human Services is REQUIRED.
