We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Per Diem team at Chestnut Woods Rehab.
Job Description:
As an LPN with Chestnut Woods Rehab, you will have the opportunity to work with patients in accordance with acceptable standards of nursing practice and the written plan of care. You will be responsible for delivering patient care, following a plan of care designed by Nurse Manager, and maintaining accurate documentation.
Responsibilities:
- Deliver patient care in accordance with acceptable standards of nursing practice and the written plan of care.
- Follow a plan of care designed by Nurse Manager.
- Perform medication administration and treatments per plan of care.
- Monitor patients' conditions and report accordingly.
- Maintain accurate documentation in accordance with orders provided.
Qualifications:
- A current, unencumbered active license to practice as an LPN in the state of MA is required.
- The ability to work with multiple patients at once is essential.
- The primary series of the COVID-19 vaccine is required.
