A Day in the Life of an RN Care Manager:

The RN Care Manager assists patients and their caregivers to ensure safe and effective transfers across the continuum of care and educate patients to assist them in their healthcare decisions.  The RN Care Manager serves as patient advocate, patient empowerment facilitator, and provide education and guidance for effective care transitions, improved self-management skills, and enhanced patient-professional communication.

 Here Is What You Will Be Doing:

  • Works collaboratively with physicians, hospital and clinical staff to track progress of patients and identify opportunities to assist in improving quality outcomes.  Monitors patient’s response to treatment, works with patient’s team to analyze and address unanticipated developments, and assists in removing barriers to care.
  • interventions of high-risk patients in the hospital and/or PCMH/PCP environment.  
  • Provides continuity of care management for those patients with multiple chronic conditions.  
  • Assists in leading multidisciplinary rounding, collaborating care and assist in management of patient’s length of stay.
  • Builds trust and rapport with patient and/or primary caregiver, while assisting patient and/or primary caregiver to set goals for treatment and general health; provides post-discharge visits and phone follow up to patients who meet program criteria.
  • Provides information and bring best practice to patient education, and support to patient’s and/or primary caregiver regarding disease(s) self-management.  
  • Involves patient and support person in pre-op education class & activities, post–operative education and discharge instructions to assure understanding for level of care transition.  
  • Prepares patient to make post discharge informed decisions for placement.
  • Provides accurate, on-going documentation of care coordination activities to include care oversight, case management and discharge planning needs. 
  • Assists patient and/or primary caregiver to understand, create, and utilize a personal health record, including assistance in formulating key questions and/or concerns to ask health care providers.  
  • Provides point of contact for internal and external communication resources for patient and families.
  • Develops standardized care plans with PCMH/PCP/Specialty providers; including detailed discharge orders, medication instructions and follow-up care.  
  • Assists in managing care transitions between and among all providers and settings using electronic transmission of information.

  • Ability to assess a patient’s condition, formulate plan of care, select appropriate interventions, evaluate patient’s response to plan of care and to explain to patient/family about this process
  • Ability to assess, evaluate, explain, teach, problem solve using critical thinking; and perform calculations
  • Excellent professional/clinical skills and utilization of the Nursing process of direct and indirect patient care which includes assessment, planning, implementation and evaluation of nursing care to meet patient needs and improve patient outcomes
  • Ability to work independently while collaborating with other team members
  • Ability to train nurses to emulate a care transitions model in order to have expected outcomes
  • Knowledge and appreciation of cultural diversity and low literacy issues, and how those factors can affect coaching and health care provision
  • Knowledge of hospital structure, nursing policy and procedures, and chronic disease management
  • Ability to drive to and from a variety of settings in varying weather conditions

 

Here Is What You Will Need:

  • Wyoming Registered Nurse (RN) License or enhanced Nurse Licensure (eNLC)
  • Three (3) or more years of experience in acute care nursing
  • Valid USA driver’s license
  • Must have clean driving record that follows Cheyenne Regional Medical Center’s driving guidelines
  • Cheyenne Regional AHA RQI within 14 calendar days of start date

 

 Nice to Have:

  • Bachelor’s degree or higher from an accredited school of nursing
  • Home Health Care experience
  • Department specific experience
