- Coordinate and link patients with additional services
- Provide crisis intervention, stabilization, and brief counselling as needed
- Provide proactive behavioral health case management to clinic patients
- Employed position
- Full-time Monday through Friday work schedule
- Fluency in an Asian language(s) or Spanish is preferred but not required
- Ability to work with limited English proficient populations
- Master's degree in related fields
