Post-Acute Care Coordination
Our goal is to provide comprehensive care coordination across the continuum of care. As a Post-Acute Care Coordinator, you will work closely with hospital partners to identify and prioritize patient populations who will benefit most from home-based services, such as home health or hospice.
You will leverage your knowledge of post-acute care delivery to address and navigate barriers within the healthcare system, ensuring patients have access to appropriate home-based care. By doing so, you will contribute to enhanced patient outcomes, improved satisfaction, and reduced care inefficiencies.
