Job Title : Social Worker
Job Specialty : Social Work - Home Health / Hospice
Job Duration : 13 weeks
Shift : 12H Days, Monday to Friday, 8 AM to 5 PM
Guaranteed Hours : 40 hours per week
Experience : Minimum of 1-2 years (no new graduates)
License : ND Social Worker License
Certifications : BLS, and one of the following : LMSW-AP, MSW, LCSW, CHP-SW
Must-Have : Critical thinking skills, flexibility, knowledge of death and dying
Job Description :
- Provide hospice admission assessments and bereavement assessments
- Develop plans of care and follow up on family and client social needs
- Assist with the social needs of home health clients as needed
- Preferred experience with HomeCare HomeBase EMR system
- Work within hospice and home care services, covering assigned areas
- Consult with an available SW Manager / Supervisor as needed
- Attend a department-specific orientation; expedited based on travel position
